Physical Therapist Job Requirements

- Ability to work collaboratively with interdisciplinary care teams
Responsibilities
- Evaluate and treat patients according to individualized physical therapy care plans
- Provide PT services focused on improving mobility, strength, function, balance, and safety
- Complete accurate and timely OASIS documentation as required
- Monitor patient progress and adjust treatment plans based on clinical findings
- Educate patients and caregivers on exercises, mobility strategies, and ongoing care needs
- Collaborate with physicians and other healthcare professionals to support positive patient outcomes
