Key Qualifications
- Current Physical Therapy licensure in the state of South Carolina (required)
- Current CPR certification
- Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
- Associate’s Degree or higher in Physical Therapy
- At least 1 year of experience as a Physical Therapist (home health experience preferred)
- Ability to function professionally in diverse home environments
- Solid color scrubs required
Primary Responsibilities (Home Health Physical Therapy)
- Evaluate patients’ functional status, mobility, and physical therapy needs in the home setting
- Develop and assist physicians and interdisciplinary teams with therapy plans of care for home health patients
- Provide skilled physical therapy services in accordance with the plan of care and professional scope of practice
- Deliver patient-centered rehabilitation care to relieve pain, restore function, and maximize independence
- Educate patients, families, and caregivers on home exercise programs, functional limitations, safety, and progress toward goals
- Coordinate care with other healthcare team members, including nurses, occupational therapists, and case managers
- Document patient assessments, treatment plans, progress notes, and outcomes accurately in the EMR per agency policy and Medicare guidelines
- Observe and report patient responses to treatment, any changes in condition, and anticipated frequency of care
