Key Qualifications

• Current Physical Therapy licensure in the state of South Carolina (required)
• Current CPR certification
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation
• Associate’s Degree or higher in Physical Therapy
• At least 1 year of experience as a Physical Therapist (home health experience preferred)
• Ability to function professionally in diverse home environments
• Solid color scrubs required

Primary Responsibilities (Home Health Physical Therapy)

• Evaluate patients’ functional status, mobility, and physical therapy needs in the home setting
• Develop and assist physicians and interdisciplinary teams with therapy plans of care for home health patients
• Provide skilled physical therapy services in accordance with the plan of care and professional scope of practice
• Deliver patient-centered rehabilitation care to relieve pain, restore function, and maximize independence
• Educate patients, families, and caregivers on home exercise programs, functional limitations, safety, and progress toward goals
• Coordinate care with other healthcare team members, including nurses, occupational therapists, and case managers
• Document patient assessments, treatment plans, progress notes, and outcomes accurately in the EMR per agency policy and Medicare guidelines
• Observe and report patient responses to treatment, any changes in condition, and anticipated frequency of care
