Home Health Registered Nurse (RN) - Contract Job in Newton, MA

- Schedule: 8-hour day shifts, 40 hours per week
- Weekend Rotation: Approximately once every 6 weeks
- Weekly Estimated Pay: $2,500 - $2,630
Registered Nurse Job Requirements
- Active Registered Nurse (RN) license
- At least 1 year of certified home care experience required
- At least 1 year of case management experience required
- Ability to travel to assigned patient cases as needed
- Strong electronic documentation skills and comfort with interdisciplinary communication
- Experience working within community health or home-based care settings preferred
Home Health Nursing Responsibilities
- Assess patient conditions and provide skilled nursing care in the home setting
- Develop and implement individualized care plans based on patient needs
- Perform admission assessments and case management responsibilities
- Provide ongoing education and support to patients and their families
- Document care thoroughly and in a timely manner using electronic systems
- Communicate effectively with the interdisciplinary care team to support coordinated care
- Follow organizational policies, standards of practice, and community health guidelines
