- Shift: 8-hour days (8:00am – 4:30pm), Monday–Friday
- Hours per Week: 40
- On-Call: Average one weekend per month; 1–2 overnight shifts per week (4:30pm – 8:00am)
- Holiday Rotation: Yes, rotating
- Estimated Weekly Pay: $2,500 – $2,630
Job Requirements / Qualifications
- Active Registered Nurse (RN) license; AK license preferred — must apply for AK license within 24 hours of accepting offer
- BLS (Basic Life Support) certification from AHA required
- Minimum 1 year of experience in a CMS/OASIS Home Health program required; 2 years preferred
- Experience with OASIS documentation, home health assessments, and care coordination
- Reliable transportation for home visits required
Responsibilities
- Provide skilled nursing care to patients in their homes following individualized home health care plans
- Perform comprehensive OASIS assessments and complete timely, accurate documentation
- Coordinate care with interdisciplinary team members (PT, OT, SLP, social work) to optimize patient outcomes
- Participate in on-call rotation for after-hours and weekend coverage as scheduled
- Educate patients and families on disease management, medications, wound care, and home safety
- Maintain compliance with regulatory requirements and agency policies
- Support care transitions and discharge planning to reduce hospital readmissions
