• Shift: 8-hour days (8:00am – 4:30pm), Monday–Friday
• Hours per Week: 40
• On-Call: Average one weekend per month; 1–2 overnight shifts per week (4:30pm – 8:00am)
• Holiday Rotation: Yes, rotating
• Estimated Weekly Pay: $2,500 – $2,630

Job Requirements / Qualifications

• Active Registered Nurse (RN) license; AK license preferred — must apply for AK license within 24 hours of accepting offer
• BLS (Basic Life Support) certification from AHA required
• Minimum 1 year of experience in a CMS/OASIS Home Health program required; 2 years preferred
• Experience with OASIS documentation, home health assessments, and care coordination
• Reliable transportation for home visits required

Responsibilities

• Provide skilled nursing care to patients in their homes following individualized home health care plans
• Perform comprehensive OASIS assessments and complete timely, accurate documentation
• Coordinate care with interdisciplinary team members (PT, OT, SLP, social work) to optimize patient outcomes
• Participate in on-call rotation for after-hours and weekend coverage as scheduled
• Educate patients and families on disease management, medications, wound care, and home safety
• Maintain compliance with regulatory requirements and agency policies
• Support care transitions and discharge planning to reduce hospital readmissions
