Job Requirements / Qualifications

• Active Certified Nursing Assistant (CNA) certification in the United States
• Minimum 1 year of hospice experience within the past 3 years (Hospice CNA experience required)
• Experience with Homecare Homebase (HCHB) Electronic Medical Record (EMR) system required
• Ability to travel within South King County and support other locations as needed (travel CNA duties)
• Strong communication, teamwork, and documentation skills; commitment to patient-centered care

Responsibilities (Hospice CNA / Clinical Support)

• Provide compassionate, hands-on care to patients in a hospice/home health setting as a Certified Nursing Assistant (CNA)
• Assist patients with activities of daily living (ADLs) including bathing, dressing, feeding, toileting, and mobility support
• Monitor and report changes in patient condition to the nursing team and interdisciplinary care providers
• Document care and interventions accurately using the HCHB EMR system
• Support safe patient transfers, positioning, and mobility; follow infection control and hospice/home health safety protocols
• Collaborate with nurses, therapists, and other healthcare professionals to ensure high-quality, coordinated care
• Travel to assigned sites within the Tukwila/South King County territory as needed to meet patient care needs
