- Coverage Areas: Fayette, Jessamine, Franklin, Woodford, Anderson, and Mercer counties
- Productivity Expectation: 30 points per week
- Mileage Reimbursement: Yes, between visits
- Contract Duration: 22 weeks
Job Requirements
- Active Occupational Therapist license
- CPR certification
- Bachelor's degree required
- At least 1 year of Occupational Therapy experience preferred
- Home health and OASIS experience strongly preferred
- Experience using HCHB preferred
- Strong clinical judgment, organization, and communication skills
- Ability to work independently in a home health environment
Responsibilities
- Complete home health Occupational Therapy evaluations and follow-up treatment visits
- Develop and implement individualized care plans based on patient needs
- Perform OASIS documentation accurately and in a timely manner
- Collaborate with the interdisciplinary care team to support patient progress
- Educate patients and caregivers on therapy goals, safety, and home exercises
- Maintain productivity standards while delivering high-quality patient care
- Travel between patient visits within the assigned coverage area
