Job Requirements
- Doctor of Pharmacy (PharmD) or BS Pharmacy degree required
- Active Texas pharmacist license required
- Hospital experience preferred
- BLS or ACLS certification required (AHA only)
- PALS required for pediatric practice (AHA only)
- Epic experience highly preferred
- Experience with pharmacokinetic dosing preferred
- Neonatal medication knowledge and emergency medicine experience preferred
Responsibilities
- Prepare and dispense medication orders accurately and efficiently in a hospital pharmacy environment
- Interpret verbal and written medication orders and document them in the patient record
- Compound sterile and non-sterile medications, including chemotherapy and parenteral nutrition products
- Monitor patient drug therapy for appropriateness, interactions, contraindications, and allergies
- Support pharmacokinetic consults and renal dose adjustments per protocol
- Provide clear drug information and clinical consultation to providers and care teams
- Educate patients and families on medications when appropriate
- Supervise and direct pharmacy support personnel as needed
- Support quality improvement, medication use review, and stewardship initiatives
- Participate in interdisciplinary rounds and clinical interventions when assigned
