Job Requirements
- Bachelor's Degree from a College of Pharmacy accredited by the American Council on Pharmaceutical Education (ACPE) (required)
- Doctor of Pharmacy (PharmD) preferred — PharmD and clinical pharmacy experience are a plus
- Active Pennsylvania Pharmacist License (required)
- Basic Life Support (BLS) certification preferred
- Minimum 1 year of hospital pharmacy experience or Advanced Pharmacy Practice Experience (APPE) in a clinical setting preferred
Responsibilities
- Review all medication orders from physicians and physician extenders to ensure medication safety and appropriate therapy
- Dispense medications accurately, verifying drug, dose, frequency, and route
- Assess patient allergies, medical history, drug interactions, and laboratory data for potential clinical conflicts
- Maintain inventory control and ensure accuracy in medication preparation and distribution
- Provide drug information and clinical support to interdisciplinary healthcare team members
- Supervise and mentor pharmacy students and pharmacy technicians to promote safe medication use
- Collaborate with the care team on medication reconciliation, discharge planning, and continuity of care
