Hospital Pharmacist – Travel Contract (San Francisco, CA)
13-week Travel Pharmacist job in San Francisco (94109) — PharmD or equivalent experience accepted, California-licensed Hospital Pharmacist opportunity at a leading medical center. Join our clinical pharmacy team in the heart of San Francisco for an on-site travel contract combining patient-centered medication safety and hands-on hospital pharmacy practice.

- Contract Duration: 13 weeks (June 1, 2026 – August 31, 2026)
- Employment Type: Travel Pharmacist, On-Site Hospital Pharmacy
- Shifts: Variable; Day shifts (4 x 10-hour, 07:00–17:30) and Night shifts (5 x 8-hour, 21:00–05:30); start times range from 07:00–21:30
- Hours Per Week: 40
- Schedule Requirements: Every other weekend required; no on-call
Job Requirements
- Doctor of Pharmacy (PharmD) from an accredited school of Pharmacy or equivalent experience
- Licensed Pharmacist (PHARMD or PHARMR) with a valid California pharmacist license at time of submission
- Minimum 2 years of hospital pharmacy experience required (hospital pharmacy jobs / clinical pharmacist experience)
- Completion of PGY1 Pharmacy Residency Program strongly preferred (pharmacy residency / clinical pharmacy)
- Epic charting experience preferred (EPIC, electronic health record experience)
- All certifications must be AHA and in hand at time of submission
- Ability to provide accurate 7-year work history verification
- Must be available for lab work within 3-5 days of signing
- Travelers may float to other locations if unit census is low
Responsibilities
- Deliver pharmaceutical care through patient-centered clinical pharmacy services, medication information, education, and medication preparation/distribution
- Ensure safe, effective, and cost-efficient medication therapy and medication safety practices
- Exercise sound clinical judgment and implement patient care plans based on diagnosis and interdisciplinary collaboration
- Provide medication reconciliation, dosing recommendations, and therapeutic monitoring in a hospital setting
- Foster confidence and cooperation from patients, families, and healthcare providers through competent assessment, monitoring, and clear communication
- Adhere to all local, state, and federal regulations, codes, policies, and procedures to ensure privacy and safety
- Perform specific pharmacy tasks and orient or mentor other staff as needed
- May float to other units or locations as required (travel hospital pharmacist flexibility)
