Job Details

• Position: Travel Physical Therapist (PT) – Home Health (Travel PT / home health physical therapy)
• Location: Burbank, CA 91502 (Los Angeles area, Southern California)
• Assignment Length: 11 weeks (March 30, 2026 – June 14, 2026)
• Estimated Weekly Pay: $2,310 – $2,480
• Schedule: Monday – Friday, 8:00 AM – 5:00 PM
• Shift: 8-hour days, 40 hours/week
• Employment Type: Travel Contract (Travel PT)
• On-Call / Weekend Requirements: None

Job Requirements & Qualifications

• Active Physical Therapist (PT) license required to practice in California
• Experience with HCHB (Homecare Homebase) EMR required
• Strong clinical assessment, documentation, and organization skills
• Ability to work independently and collaboratively within a multidisciplinary team (physicians, nurses, therapists)
• Comfort treating a variety of patient populations, including Women's Health, Vestibular, and Pelvic Rehab cases

Key Responsibilities

• Deliver high-quality physical therapy services in a home health setting (home health physical therapy)
• Assess patients, develop individualized care plans, and implement evidence-based interventions
• Manage daily productivity expectations (approximately 5 points per day; combination of follow-up and start-of-care visits)
• Accurately document all patient care activities and progress in HCHB EMR
• Collaborate with physicians, nurses, and rehabilitation team members to coordinate patient care
• Treat patients with varied needs including Women's Health, Vestibular, Pelvic Rehab, and general home health rehabilitation
