Travel Physical Therapist (Home Health & Hospital) – Fort Morgan, CO (13-Week Assignment)

• Weekly Estimated Pay: $1,740 – $1,870
• Location: Fort Morgan, Colorado (80701) — Fort Morgan healthcare & Colorado travel PT jobs; serving surrounding areas within a 50-mile radius
• Assignment Duration: 13 weeks (Start: 4/20/2026 – End: 7/19/2026)
• Role: Travel Physical Therapist — Home Health & Hospital
• Shift: 8-hour shifts, Monday–Friday (08:00 AM – 04:30 PM)
• Hours per Week: 40 (Guaranteed)
• Dress Code: Blue/Black scrubs
• Travel: Daily travel to patient homes and agency service locations (home health travel required)

Job Requirements / Qualifications

• Master's or Doctorate degree in Physical Therapy (MPT or DPT required)
• Active, unencumbered Physical Therapist license in Colorado (Licensed PT)
• Minimum 1 year of experience as a Physical Therapist with Home Health OASIS experience required
• Strong documentation skills (OASIS, physician orders) and electronic charting experience
• Ability to travel daily within a 50-mile radius for home health visits
• Excellent communication, patient education, and care coordination skills
• Preferred: Experience with Homecare Homebase (HCHB) or similar home health EMR

Primary Responsibilities

• Provide skilled physical therapy services in both hospital and home health settings (home health physical therapist, hospital-based PT)
• Conduct comprehensive patient evaluations and develop individualized, goal-directed care plans
• Perform skilled assessments of mobility, strength, balance, gait, pain, endurance, and functional limitations
• Evaluate home environments for fall risks, safety barriers, and equipment needs
• Deliver evidence-based interventions including therapeutic exercise, gait and balance training, transfer and stair training
• Instruct patients and caregivers on proper use of assistive devices and home exercise programs, fall prevention, and energy conservation
• Modify treatment plans based on medical complexity and patient response; prioritize safe, functional outcomes
• Coordinate care with physicians and interdisciplinary team members (RN, OT, SLP) and complete timely documentation (OASIS)
• Plan and implement safe discharge plans, home maintenance programs, and referrals as needed
