Job Requirements

• Experience: Minimum 1 year of experience as a Physical Therapist in a hospital or inpatient setting (inpatient PT experience preferred)
• License: Active Arizona Physical Therapist license required (compact license acceptable to start, but permanent AZ license must be obtained promptly)
• Certification: Licensed Physical Therapist (PT)
• Compliance: Ability to complete all onboarding and credentialing documentation prior to start
• Availability: Able to work full-time (40 hours per week) on a daytime schedule

Responsibilities

• Evaluate and treat adult and/or pediatric patients with a variety of acute diagnoses in an inpatient hospital setting
• Develop and implement individualized care plans and therapeutic interventions to improve mobility, strength, balance, and function
• Collaborate with physicians, nurses, case managers, and the multidisciplinary care team to coordinate treatment and discharge planning
• Document evaluations, progress notes, plan of care updates, and discharge summaries in accordance with hospital policies and clinical documentation standards
• Educate patients and families on home exercise programs, fall prevention, mobility strategies, and safe transitions of care
• Recommend adaptive equipment and support safe discharge planning, including outpatient or home health referrals as appropriate
