We are seeking an experienced Licensed Practical Nurse (LPN) for a 13-week homecare travel assignment in Fort Worth, Texas. This on-site homecare opportunity is ideal for a compassionate clinician who enjoys delivering skilled nursing care in patients' homes while providing patient education, support, and high-quality clinical care. - Schedule: Monday-Friday, 8:00 AM-4:30 PM
- Shift Duration: 8 Hour Days
- Shifts Per Week: 5
- Hours Per Week: 40
- Duration: 13-week assignment
- Weekend Requirement: One weekend per month
Job Requirements
- At least 1 year of home health field clinician experience required
- Active Licensed Practical Nurse (LPN) license
- Active driver's license
- Current CPR certification
- Ability to provide skilled nursing care in a homecare setting
- Homecare Homebase (HCHB) experience preferred
Responsibilities
- Provide skilled homecare visits to patients throughout the Fort Worth service area
- Deliver patient education to support care plans, disease management, and health outcomes
- Perform wound care, including wound vacs
- Complete venipuncture and assist with PICC line-related care
- Support care involving pleurX drains and other skilled nursing procedures
- Document care accurately and communicate effectively with the interdisciplinary healthcare team
- Maintain professionalism, compassion, and patient-centered care during each home visit
