Job Requirements
- Active Registered Nurse (RN) license
- Minimum of 2 years of bedside clinical experience
- ICU experience or strong clinical skills appropriate for LTACH care
- BLS certification required prior to first shift
- ACLS certification required prior to first shift
- Strong documentation, communication, and critical thinking skills
- Ability to work effectively in a fast-paced, patient-focused healthcare environment
Responsibilities
- Complete a thorough nursing assessment for each patient within 8 hours of admission
- Develop and follow individualized plans of care in collaboration with the interdisciplinary team
- Provide direct patient care using the nursing process of assessing, planning, implementing, and evaluating
- Support patient rehabilitation goals and promote positive outcomes through patient and family education
- Initiate discharge planning from the day of admission and provide teaching in anticipation of discharge
- Communicate timely updates regarding patient status to physicians, charge nurses, and team members
- Document assessments, interventions, and patient responses accurately and in a timely manner
- Supervise and guide LPNs, CNAs, and other support staff as appropriate
- Promote a safe environment for patients, visitors, and staff while following all safety policies and procedures
- Contribute to team collaboration, unit efficiency, and professional growth
