Job Requirements / Qualifications

  • Active Registered Nurse (RN) license in the United States (state-specific licensure preferred)
  • BLS Certification (required prior to first shift)
  • ACLS Certification (required prior to first shift)
  • Minimum of 1 year of recent bedside clinical experience; 2 years preferred
  • Experience in LTAC, long-term acute care, ventilator care, step-down, or similar acute care settings is preferred
  • Strong clinical assessment, documentation, and care-planning skills

Responsibilities

  • Perform comprehensive nursing assessments within 8 hours of patient admission and develop individualized LTAC care plans
  • Provide direct patient care for medically complex patients, including ventilator care, tracheostomy management, wound care, and skilled nursing needs
  • Collaborate with interdisciplinary teams (physicians, therapists, case managers) to set rehabilitation goals and coordinate continuity of care
  • Document assessments, interventions, and patient responses accurately and in a timely manner according to facility and regulatory requirements
  • Initiate discharge planning and patient/family teaching from the day of admission to support safe transitions of care
  • Communicate changes in patient status to the care team to ensure patient safety and quality outcomes
  • Supervise and provide direction to Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), and orientees as needed
  • Promote and maintain a safe clinical environment by following infection control and safety protocols
  • Assume charge nurse responsibilities after orientation when needed and contribute to unit-based quality improvement initiatives
  • Engage in ongoing professional development and continuing education to maintain LTAC nursing competencies
