Long Term Care Nurse Job Details

  • Location: Berlin, Vermont (05602) – Vermont nursing jobs
  • Healthcare Setting: Long Term Care – Maple Grove Unit
  • Assignment Length: Up to 32 weeks
  • Start Date: November 24, 2025
  • End Date: March 15, 2026
  • Nursing Shift: 12-hour nights
  • Hours per Week: 36
  • Schedule: Every other weekend and 2 out of 4 major holidays (Thanksgiving, Christmas, Christmas Eve, New Year's Day)
  • Travel Nursing Assignment

LPN Qualifications & Requirements

  • Active Licensed Practical Nurse (LPN) license – Vermont LPN jobs
  • BLS certification through AHA
  • Minimum of 3 years of LPN experience in healthcare
  • Prior travel nursing experience required
  • Long Term Care (LTC) experience required
  • Experience caring for dementia patients
  • Ability to float to other units as needed
  • Reliable personal or rental vehicle required (no public transportation available)
  • Vehicles must have all-season or snow tires November–April

Long Term Care Nurse Responsibilities

  • Deliver high-quality nursing care to residents in a long-term care healthcare setting
  • Administer medications and treatments as prescribed
  • Monitor and document patient status and progress
  • Collaborate with interdisciplinary healthcare team members for comprehensive patient care
  • Provide compassionate support to residents with dementia and chronic conditions
  • Float to other units as needed to support clinical care
  • Adhere to facility policies, procedures, and assigned schedules
  • Maintain clear communication regarding scheduling and preplanned time off
