Job Requirements
- Active LPN license in the state of New Hampshire (LPN - NH)
- Minimum 1 year of LPN experience; long-term care or skilled nursing facility experience preferred
- Ability to work 12-hour day shifts and rotating schedules
- Strong communication, teamwork, and resident-centered care skills
- Commitment to compassionate patient care and documentation accuracy
Responsibilities
- Provide direct nursing care to residents in a long-term care/skilled nursing setting
- Administer medications and treatments as prescribed and document medication administration
- Monitor and record vital signs, resident progress, and changes in condition
- Collaborate with the interdisciplinary team (RNs, CNAs, therapists, social work) to ensure quality care and care plan updates
- Assist and support residents with activities of daily living (ADLs) and promote resident independence and well-being
- Maintain a safe, clean, and compliant care environment for residents and staff
