Job Requirements / Qualifications
- Minimum 1 year of experience as a Physical Therapist Assistant (PTA)
- At least 6 months of Home Health experience as a PTA or in-home therapy
- HCHB (Homecare Homebase) experience required — experience documenting home health physical therapy visits
- Current Massachusetts PTA licensure (licensed PTA in MA)
- Current CPR certification
- Valid driver’s license, vehicle insurance, and access to a dependable vehicle or reliable public transportation for travel within the Westborough area
Responsibilities
- Provide high-quality, patient-centered physical therapy services in patients’ homes under indirect supervision
- Deliver in-home therapy interventions, therapeutic exercise instruction, and functional mobility training for adult and geriatric patients
- Document patient progress and care plans using HCHB software accurately and timely
- Collaborate with interdisciplinary teams including physicians, home health nurses, case managers, and rehab directors
- Educate patients and families on therapeutic exercises, fall prevention, and home safety modifications
- Manage a travel schedule and travel within the Westborough area to ensure timely patient visits as a mobile PTA
