Physical Therapist Qualifications

• Active Physical Therapist (PT) license in Pennsylvania
• Minimum 1 year of experience as a Physical Therapist; home health or homecare experience preferred
• Strong clinical skills in hospital-based therapy, women's health, vestibular rehabilitation, pelvic rehab, and case management
• Excellent communication and organizational skills
• Ability to work independently and travel within assigned Philadelphia neighborhoods

Physical Therapist Responsibilities

• Deliver comprehensive physical therapy services to patients in their homes throughout Philadelphia
• Develop and implement individualized treatment plans based on patient needs and rehabilitation goals
• Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes
• Educate patients and families on therapy techniques, safety, and wellness best practices
