Home Health PTA Job Details

• Assignment: 13-week travel contract, November 30, 2026–March 1, 2027
• Setting: Home health; visits take place in patients’ homes and care facilities, with periodic office meetings
• Schedule: Typically Monday–Friday, 8:00 a.m.–5:00 p.m.; 5 shifts per week, 8 hours per shift
• Weekly Estimated Pay: $1,370–$1,470
• Location and Territory: Gray and Monroe County, Georgia; willingness to travel up to 75 miles from the assigned care center is required
• Workload: Typically 5–10 assigned patients per day, with a target of 30 productivity points per week
• Hours: Hours are not guaranteed and depend on patient census. If needed, opportunities to pick up visits at a nearby clinic may be discussed.
• Travel: Mileage is reimbursed at the standard IRS rate for travel between patient homes; travel to the first and from the final visit is not included.
• Additional Scheduling: Weekend and on-call expectations may vary by territory and should be discussed during the interview. Overtime requires prior approval.

Physical Therapist Assistant Job Requirements

• At least 1 year of recent adult home health experience; first-time home health clinicians are not being considered.
• Current Georgia Physical Therapist Assistant (PTA) license in hand.
• Current CPR/BLS certification.
• Proficiency with HomeCare HomeBase; recent use within the past 6 months is expected, or a refresher may be required.
• Reliable personal vehicle, valid driver’s license, and required auto insurance.
• Comfort traveling throughout the assigned territory, accepting scheduled patient visits, and attending periodic office meetings.
• Ability to complete visit documentation within 24 hours and meet the weekly productivity target.

Home Health Physical Therapist Assistant Responsibilities

• Provide physical therapy assistant services to adult patients in home and care-facility settings under the direction of a physical therapist.
• Complete start-of-care and follow-up visits, supporting individualized therapy plans and patient progress.
• Monitor and document patient response, progress, and relevant clinical observations in HomeCare HomeBase.
• Complete visit documentation within 24 hours and coordinate care with the appropriate clinical team.
• Travel between assigned patient locations and manage a daily schedule across the Gray and Monroe County service area.
