Travel Physical Therapist (PT) – Home Health Contract in Tallahassee, Florida
Travel Physical Therapist job in Tallahassee, FL: provide skilled home health care across five counties with guaranteed 40-hour weeks. Evaluate patients in their homes, deliver personalized physical therapy, and help patients improve mobility, strength, balance, and independence. This approximately 9-week travel contract covers Leon, Wakulla, Madison, Jefferson, and Taylor Counties. Enjoy Florida’s capital city, with inviting parks, local arts, and outdoor spaces.
Home Health Physical Therapist Job Details
- Setting: Home health physical therapy
- Location: Tallahassee, Florida; coverage across Leon, Wakulla, Madison, Jefferson, and Taylor Counties
- Contract: Travel assignment, approximately 9 weeks
- Assignment Dates: November 11, 2026 – January 11, 2027
- Estimated Weekly Pay: $2,030–$2,190
- Hours: 40 guaranteed hours per week
- Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.; 8-hour shift duration
- On-Call and Weekends: No on-call requirement; weekend work is optional
- Travel: Mileage reimbursed between patient visits
- Productivity Expectation: 30 points
Physical Therapist Job Requirements
- Current, valid Florida Physical Therapist (PT) license
- At least 1 year of physical therapy experience, including home health experience
- Doctoral-level education as required for the position
- Experience with OASIS documentation
- Ability to travel throughout the assigned coverage area
- Self-directed, personable approach and strong communication skills
Home Health Physical Therapist Responsibilities
- Evaluate patients in their homes and identify functional limitations, mobility needs, and physical therapy goals.
- Develop and deliver individualized home health physical therapy plans to support safe movement, strength, balance, and independence.
- Monitor patient progress, adjust treatment as appropriate, and document services and outcomes accurately, including required OASIS documentation.
- Educate patients and caregivers on therapy activities, safe mobility, and use of recommended techniques or equipment.
- Coordinate care with other members of the clinical team and communicate changes in patient condition or progress.
- Manage a home visit schedule across the designated Florida counties while meeting productivity expectations.
