Travel Physical Therapist (PT) – Home Health | Granite City, IL
Explore a 26-week travel Physical Therapist (PT) home health contract in Granite City, Illinois, serving Madison and St. Clair counties. Provide compassionate in-home physical therapy that helps patients improve mobility and independence.
Deliver skilled physical therapy directly to patients in their homes across the greater St. Louis region and the scenic Mississippi River corridor. You’ll complete patient evaluations, develop individualized plans of care, monitor progress, and collaborate with and supervise physical therapist assistants.
Travel Physical Therapist Job Details
- Setting: Home health physical therapy
- Assignment: Travel contract, approximately 26 weeks
- Assignment dates: October 5, 2026 – April 5, 2027
- Schedule: Monday–Friday, 8:00 a.m.–5:00 p.m.; 8-hour shifts, 5 shifts per week
- Hours: 40 hours per week
- Estimated weekly pay: $2,030–$2,190
- Location and coverage area: Granite City area; Madison and St. Clair counties, Illinois
- On-call/weekend requirements: None
- Travel between visits: Mileage reimbursed between patient visits
- Productivity expectation: 25 points per week
Physical Therapist Job Requirements
- Current Illinois Physical Therapist (PT) license
- Bachelor’s degree
- At least 1 year of physical therapy experience; home health experience preferred
- Strong patient assessment, care-planning, communication, and time-management skills
- Experience with Home Care Home Base is a plus
Home Health Physical Therapist Responsibilities
- Provide in-home physical therapy services through patient visits across the assigned Illinois coverage area.
- Complete patient admissions, physical therapy evaluations, and discharge visits.
- Develop and establish individualized home health physical therapy plans of care.
- Monitor patient progress and adjust interventions to support safe, functional outcomes.
- Supervise physical therapist assistant staff and coordinate patient care as appropriate.
- Document visits and clinical progress accurately in the required care management system.
