Qualifications / Job Requirements
- Bachelor's Degree in Physical Therapy (minimum requirement); Doctor of Physical Therapy (DPT) preferred
- Active and valid Physical Therapist license (PT) in Pennsylvania
- Minimum 1 year of physical therapy experience; home health experience preferred
- Experience with Homecare Homebase (HCHB) and OASIS documentation is a plus
- Strong clinical assessment and manual therapy skills in a home health/rehab setting
- Valid driver's license, reliable vehicle, and ability to travel between patient homes
- Excellent communication skills and ability to collaborate with interdisciplinary teams
Responsibilities – Home Health Physical Therapist
- Provide high-quality, patient-centered home physical therapy services to adults in Mifflin and Juniata Counties (Lewistown, PA)
- Conduct comprehensive evaluations, develop individualized treatment plans, and perform progress assessments
- Deliver evidence-based therapeutic interventions focused on mobility, functional independence, pain management, and fall prevention
- Document patient care accurately and timely using required systems (OASIS/HCHB when applicable)
- Coordinate care with nurses, occupational therapists, speech therapists, and primary care providers to optimize outcomes
- Travel between patient homes within the assigned service area; mileage reimbursement provided
- Educate patients and caregivers on home exercise programs and safe functional strategies
