Travel Physical Therapist – Home Health | Sebring, Florida
Travel Physical Therapist job in Sebring, FL: deliver home health care, complete OASIS visits, and support patients across Highlands and Hardee Counties.

This approximately 12-week home health travel assignment offers a Monday–Friday schedule, mileage reimbursement, and no weekend or on-call requirements. Travel Physical Therapist Job Details
- Location: Sebring, Florida; service area includes Highlands and Hardee Counties
- Setting: Home health physical therapy
- Contract: Travel assignment, approximately 12 weeks
- Assignment dates: October 12, 2026–January 4, 2027
- Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m.; 8-hour shifts, 5 shifts per week
- Hours: 40 hours per week
- Weekly estimated pay: $2,030–$2,190
- Travel between patient visits: Mileage reimbursement provided
- Weekend and on-call requirements: None
Home Health Physical Therapist Requirements
- Current, active Physical Therapist license applicable to practice in Florida
- Bachelor’s degree in Physical Therapy or the required equivalent qualifying education
- At least 1 year of home health experience
- Experience with Homecare Homebase (HCHB)
- Ability to travel between patient homes throughout the assigned service area
Physical Therapist Responsibilities
- Provide skilled physical therapy services to patients in their homes across the assigned Florida service area.
- Complete OASIS visits and accurately document patient assessments, progress, and care provided.
- Develop and carry out individualized home health treatment plans based on each patient’s needs and goals.
- Monitor patient response to therapy and adjust interventions as appropriate.
- Coordinate care and communicate with patients, caregivers, and other members of the care team.
- Manage a schedule with an expected productivity level of 30 points.
