  • Assignment Length: 13 weeks
  • Schedule: 5 shifts per week
  • Shift Duration: 8 Hour Days
  • Hours: 40 hours per week
  • Weekend Requirement: Must work 2 weekends per month
  • Holiday Requirement: Must be available to work at least one of the following holidays: Thanksgiving, Friday after Thanksgiving, Christmas, or New Year's Day

Job Requirements

  • Active Registered Nurse (RN) license in the state of California
  • BLS certification
  • At least 3 years of acute care nursing experience
  • Previous case management experience in a hospital or acute care setting preferred
  • Strong communication, care coordination, and interdisciplinary collaboration skills
  • BSN, MSN, CCMC or ACM certification, and bilingual skills preferred

Responsibilities

  • Coordinate patient care plans across the continuum of care
  • Assess patient needs, discharge planning requirements, and available support resources
  • Collaborate with physicians, nurses, social workers, and ancillary staff to support timely and appropriate care transitions
  • Monitor patient progress and help remove barriers to discharge and treatment goals
  • Advocate for patients and families while promoting quality, safety, and efficient resource utilization
  • Maintain accurate and timely documentation in accordance with facility and regulatory standards
