- Weekly Estimated Pay: $2,296 - $2,551
- Location: Alameda, California
- Setting: On-site Case Management
- Start Date: 8/31/2026
- Assignment Length: 13 weeks
- Schedule: 5 shifts per week
- Shift Duration: 8 Hour Days
- Hours: 40 hours per week
- Weekend Requirement: Must work 2 weekends per month
- Holiday Requirement: Must be available to work at least one of the following holidays: Thanksgiving, Friday after Thanksgiving, Christmas, or New Year's Day
Job Requirements
- Active Registered Nurse (RN) license in the state of California
- BLS certification
- At least 3 years of acute care nursing experience
- Previous case management experience in a hospital or acute care setting preferred
- Strong communication, care coordination, and interdisciplinary collaboration skills
- BSN, MSN, CCMC or ACM certification, and bilingual skills preferred
Responsibilities
- Coordinate patient care plans across the continuum of care
- Assess patient needs, discharge planning requirements, and available support resources
- Collaborate with physicians, nurses, social workers, and ancillary staff to support timely and appropriate care transitions
- Monitor patient progress and help remove barriers to discharge and treatment goals
- Advocate for patients and families while promoting quality, safety, and efficient resource utilization
- Maintain accurate and timely documentation in accordance with facility and regulatory standards
