Qualifications & Job Requirements
- Active RN license (Alabama or Nursing Licensure Compact)
- At least 1 year of recent experience in Case Management, RN Case Manager, or a related clinical case management role
- Experience with Cerner EMR preferred
- Comfort managing a 1:25 patient ratio and performing utilization review tasks
- Ability and flexibility to float to different units as needed, potentially daily
- Strong communication, organizational, critical thinking, and patient advocacy skills
Key Responsibilities
- Coordinate and oversee patient care plans to ensure optimal clinical outcomes and efficient resource utilization
- Collaborate with interdisciplinary teams to facilitate patient transitions, discharge planning, and smooth care coordination
- Perform utilization management, clinical documentation, and appeals as appropriate
- Advocate for patients and families by providing education, support, and clear discharge instructions throughout the care continuum
- Maintain accurate and timely documentation in Cerner EMR
- Adapt quickly to new units and workflows while floating to meet facility needs
