RN Case Management Job Requirements
- Active California RN license
- Current BLS certification
- At least 1 year of RN Case Management experience required
- Strong knowledge of utilization management, discharge planning, and care coordination
- Ability to work collaboratively with interdisciplinary healthcare teams across the care continuum
- Excellent communication, organization, and critical thinking skills
Responsibilities
- Coordinate utilization management, resource management, and care facilitation for acute patient populations
- Lead discharge planning and post-acute care referral processes to support safe transitions of care
- Provide intervention and discharge planning coordination for high-risk patients presenting to the Emergency Department
- Promote coordinated and integrated care to help prevent unnecessary admissions and readmissions
- Support patient wellness, improved outcomes, and efficient use of healthcare services
- Assist in minimizing denials of payment through effective care planning and documentation
- Collaborate with providers, nursing staff, patients, and families to address complex healthcare needs
