Qualifications / Job Requirements
- Active RN license (Registered Nurse) in good standing
- Minimum 1 year of acute (hospital) case management or clinical case management experience
- Experience with Meditech, Midas, and Interqual systems (utilization review experience preferred)
- Strong communication, care coordination and organizational skills
- Experience in discharge planning, utilization management, and interdisciplinary collaboration
Responsibilities
- Coordinate patient care and discharge planning in an acute hospital setting as an RN Case Manager
- Use Meditech, Midas, and Interqual to manage documentation, utilization review, and case activities
- Collaborate with physicians, social workers, nursing staff and ancillary teams to ensure optimal patient outcomes
- Advocate for patients and families, providing education and support across the care continuum
- Perform care coordination tasks, resource identification, and placement facilitation for complex discharges
- Participate in weekend rotations and maintain timely, accurate case records
