Job Requirements

• Active RN license (Florida or compact state) — Registered Nurse (RN) required
• At least 1 year of recent Case Management experience (hospital case management, utilization management, care coordination)
• Experience with Meditech and Midas EMR systems (required)
• Strong communication and organizational skills for multidisciplinary collaboration
• Complex discharge planning and transitions of care experience (preferred)

Responsibilities

• Coordinate and manage patient care across the continuum to ensure optimal outcomes and safe transitions of care
• Conduct comprehensive assessments and develop individualized care plans as a Case Management RN / Case Manager
• Collaborate with interdisciplinary teams including charge nurses, CNAs, sitters, physicians, and social work
• Facilitate complex discharge planning, utilization review, and placement coordination
• Maintain accurate, timely documentation using Meditech and Midas EMR systems
• Manage a patient caseload with a ratio of at least 25:1 while prioritizing clinical needs and resource coordination
