Registered Nurse (RN) – Case Management | Rosedale, MD (On-site + Field Visits)
Registered Nurse (RN) – Case Management in Rosedale, MD: Make an immediate impact coordinating care for complex, chronic patients across Baltimore and the Washington, DC area. Join our healthcare team as a Case Manager RN focused on care coordination, discharge planning, and chronic disease management.

- Location: Rosedale, Maryland (21237) – On-site with field visits in the Washington, DC area (2 days/week)
- Start Date: May 11, 2026
- Contract Duration: 13 weeks
- Schedule: 5 x 8-hour day shifts (Monday–Friday, 8:00am–4:30pm)
- Hours per Week: 40
- Estimated Weekly Pay: $1,829 – $2,032

- On Call / Weekend Requirement: None
- Orientation: 28 hours (paid)
- Dress Code: Business casual
- Travel: Field visits required 2 days per week (mileage reimbursed)
Required Qualifications
- Active District of Columbia (DC) RN license required (pending status accepted at submission; must be active prior to start date)
- BLS certification (current)
- Minimum 2 years experience as a complex Case Manager / Case Management RN in Acute Hospital, LTAC/Rehab, or Skilled Nursing settings
- Experience with care coordination, discharge planning, utilization review, and admission criteria
- Familiarity with InterQual and Milliman Guidelines
- Regulatory knowledge: CPT codes, Department of Health DRG, HEDIS measures, NCQA, OSHA procedures, and JCC
- Strong documentation skills in EMR systems (experience with Guiding Care and/or HealthEdge preferred)
- Preferred: prior charge experience and NCQA experience; first-time travel RNs welcome with appropriate experience
Key Responsibilities
- Manage a caseload of patients with chronic and high-risk diagnoses (e.g., high-risk pregnancy/postpartum, diabetes, CHF, COPD, asthma, obesity, behavioral health)
- Perform comprehensive assessments and create individualized care plans focused on chronic disease management and care coordination
- Conduct ongoing case management, utilization review, and accurate documentation in the EMR
- Make outreach calls and perform field visits to enrollees across the Washington, DC and Baltimore areas
- Coordinate care transitions and discharge planning across multiple care settings, providers, and payors
- Ensure compliance with regulatory, accreditation, and quality standards (HEDIS, NCQA, DRG, CPT)
