Travel Nursing Job Details
- Weekly Estimated Pay: $2,118–$2,354
- Assignment Length: 13 weeks
- Start Date: November 16, 2026
- Schedule: 4 shifts per week
- Shift Duration: 10-hour day shifts
- Shift Hours: 7:00 a.m.–5:30 p.m., with rotating days off
- Expected Hours: 40 hours per week
- Weekend Requirement: Every weekend or every other weekend at a minimum
- Healthcare Setting: On-site case management supporting acute care, long-term care, and outpatient services
- Travel Nurse Eligibility: First-time travelers are welcome; applicants must be from outside the local area
Registered Nurse Case Manager Requirements
- Active Registered Nurse (RN) license eligible for practice in Utah
- At least 1 year of prior case management experience
- Experience working in acute care and long-term care settings
- Current Basic Life Support (BLS) certification
- Strong communication, organization, patient assessment, and care coordination skills
- Ability to work the required rotating schedule and weekend commitment
RN Case Manager Responsibilities
- Coordinate the case management process from patient entry into the healthcare system through post-discharge care, including outpatient services.
- Monitor patient care and progress toward desired outcomes.
- Assess and address patient and family needs throughout the care journey.
- Identify and resolve barriers to effective care and timely discharge.
- Coordinate care with payers, vendors, patients, families, and multidisciplinary internal teams.
- Establish, monitor, and execute individualized patient discharge plans.
- Access and administer medications within the scope of practice and in accordance with applicable state law, when required.
