- Duration: 13-week travel RN assignment
- Schedule: Monday-Friday, 8:00 AM-4:30 PM
- Shift Duration: 8-hour days
- Shifts Per Week: 5
- Hours Per Week: 40 hours weekly, 36 guaranteed
- On-Call/Weekend Requirement: 1 week/weekend monthly
- Coverage Area: Assigned local service area
- Mileage Reimbursement: Reimbursed between visits, from first patient to last patient
Registered Nurse Requirements
- At least 1 year of RN experience required
- Recent home health, homecare, or visiting nurse experience required
- Active Registered Nurse license in the state of Ohio
- Strong case management, care coordination, and patient assessment skills
- Ability to manage all home health visit types independently
- Home Care Home Base experience preferred
- Ability to travel throughout assigned coverage areas
Responsibilities
- Provide skilled nursing care to patients in the home setting
- Perform RN case management and coordinate individualized care plans
- Complete all home health visit types in accordance with patient needs and clinical standards
- Assess patient conditions, monitor progress, and communicate updates to the care team
- Document visits, interventions, and outcomes accurately and efficiently
- Educate patients and families on treatment plans, medications, and ongoing care needs
- Support continuity of care across the assigned service area
- Participate in on-call and weekend coverage as scheduled
