- Hours per Week: 40 (Weekly hour guarantee: 36 hours)
- Shift Schedule: Monday – Friday, 8:00 AM – 4:30 PM
- Shift Duration: 8-hour days
- On Call/Weekend Requirement: Yes, on a rotation
- Contract Duration: 13 weeks
- Mileage Reimbursement: Yes, between visits (from first patient to last patient)
Job Requirements / Qualifications
- Active Registered Nurse (RN) license (valid for practice)
- At least 1 year of nursing experience; home health experience is a plus
- Valid driver’s license and reliable transportation (required for travel RN / home visits)
- Home health experience and HCHB (Homecare Homebase) experience are a plus
Responsibilities – Home Health Nurse / Travel RN
- Provide skilled nursing care to patients in their homes within a 60-mile radius of Oklahoma City, OK 73112
- Perform comprehensive patient assessments and develop individualized care plans as a home health RN
- Deliver compassionate, evidence-based skilled nursing interventions in the home setting
- Document all care accurately and promptly (electronic documentation/HCHB when applicable)
- Coordinate with interdisciplinary team members (PT, OT, social work, physicians) to ensure optimal patient outcomes
- Participate in on-call and weekend rotations as scheduled
- Meet productivity expectations (30 points per week) while maintaining quality patient care
