Qualifications / Job Requirements
- Active Registered Nurse (RN) license in the state of New Jersey
- Minimum 1 year of nursing experience; Patient Care Coordinator, clinical coordinator, or related nursing role preferred
- Strong communication, clinical documentation, and organizational skills
- Ability to work collaboratively in a multidisciplinary healthcare team
- Experience in specialties such as Thoracic Surgery, Surgical Pathology, Sub-Acute care, Pediatric PACU, or Pediatric OR is a plus
Key Responsibilities
- Coordinate patient care activities to ensure seamless transitions across departments and services
- Advocate for patient needs and facilitate clear communication between patients, families, and healthcare providers
- Monitor patient progress, update care plans, and support clinical decision-making
- Lead and support discharge planning, follow-up care, and referral coordination
- Maintain accurate nursing documentation and uphold best practices in patient safety and quality care
