GHR Healthcare has an exciting Interim Registered Nurse Assessment Coordinator (RNAC/MDS) opening with a Continuing Care Retirement Community (CCRC) in Northampton County!

Registered Nurse Assessment Coordinator (RNAC/MDS) Job Details:
- Pay Rate: Starting at $60/hour
- Assignment Length: 6 weeks minimum with possible extension
- Schedule: In-person Monday–Friday preferred; hybrid schedule may be considered
- Shift: Day shift Preferred (evenings and weekends are also an option)
- Coordinate RAI/MDS assessments, CAAs, and documentation.
- Develop and update resident care plans.
- Collaborate with the IDT on MDS assessments and care planning.
- Audit documentation for accuracy, compliance, and coding.
- Ensure accurate MDS coding to support reimbursement.
Registered Nurse Assessment Coordinator (RNAC/MDS) Job Qualifications:
- Current, unrestricted Pennsylvania RN license or valid multistate RN license with authorization to practice in Pennsylvania.
- Resident Assessment Coordinator certification preferred
- At least 1 year of long-term care experience
- Experience with MDS/RAI and care planning processes
- Knowledge of PPS, State Case Mix, and PDPM
- Strong verbal and written communication skills
- Ability to work independently and collaborate effectively with the interdisciplinary team
