Join a high-volume inpatient hospital case management team in Birmingham, Alabama, as a Licensed Clinical Social Worker (LCSW). Job Requirements
- Active Alabama LCSW license required
- At least 1 year of recent acute care, inpatient hospital, or case management social work experience required
- Experience with psychosocial assessments, discharge planning, and patient care coordination
- Ability to work effectively in a large academic or high-volume hospital environment
- Comfort floating between units as patient care needs require
- Strong communication, teamwork, and patient advocacy skills
- Experience coordinating community resources and post-acute support services preferred
- Familiarity with Cerner preferred
Responsibilities
- Provide inpatient social work support for patients and families across assigned units
- Complete psychosocial assessments and identify barriers to care, discharge, and recovery
- Develop and support individualized care plans in collaboration with the interdisciplinary team
- Coordinate discharge planning and connect patients with appropriate community resources
- Participate in interdisciplinary rounds and communicate updates clearly with care team members
- Advocate for patient needs while promoting safe, timely, and effective transitions of care
- Adapt to unit coverage needs and float to other floors when necessary
- Maintain accurate and timely documentation within the electronic medical record
