Job Requirements

  • Active Alabama LCSW license required
  • At least 1 year of recent acute care, inpatient hospital, or case management social work experience required
  • Experience with psychosocial assessments, discharge planning, and patient care coordination
  • Ability to work effectively in a large academic or high-volume hospital environment
  • Comfort floating between units as patient care needs require
  • Strong communication, teamwork, and patient advocacy skills
  • Experience coordinating community resources and post-acute support services preferred
  • Familiarity with Cerner preferred

Responsibilities

  • Provide inpatient social work support for patients and families across assigned units
  • Complete psychosocial assessments and identify barriers to care, discharge, and recovery
  • Develop and support individualized care plans in collaboration with the interdisciplinary team
  • Coordinate discharge planning and connect patients with appropriate community resources
  • Participate in interdisciplinary rounds and communicate updates clearly with care team members
  • Advocate for patient needs while promoting safe, timely, and effective transitions of care
  • Adapt to unit coverage needs and float to other floors when necessary
  • Maintain accurate and timely documentation within the electronic medical record
