- Schedule: Monday: 8:00 AM–4:30 PM; Tuesday: 7:00 AM–4:00 PM; Wednesday: 7:00 AM–3:30 PM; Thursday: 8:00 AM–4:30 PM; Friday: 8:00 AM–4:30 PM
- Shifts per Week: 5 (8-hour days)
- Assignment Length: 13 weeks (with potential for extension) — Contract position
- Observed Holidays: New Year's Day, MLK Day, Memorial Day, Independence Day, Labor Day, Thanksgiving, Christmas
- Start Date: May 4, 2026
Job Requirements
- Active LCSW (Licensed Clinical Social Worker) license
- Minimum 1 year of experience as a Social Worker (LCSW) preferred — hospital social work or medical social worker experience desirable
- Experience with VAD (Ventricular Assist Device) cases preferred
- Heart failure and cardiac experience preferred (cardiac social work / cardiology social worker)
- Ability to work on-site in Chicago, IL (60637)
- Strong communication, psychosocial assessment, care coordination, and organizational skills
Responsibilities
- Provide comprehensive clinical social work services to patients and families, with emphasis on cardiac, heart failure, and VAD populations
- Conduct thorough psychosocial assessments and develop individualized care plans
- Collaborate with multidisciplinary teams (physicians, nurses, case management, rehab) to support patient care, discharge planning, and transitions of care
- Offer counseling, crisis intervention, and referrals to community resources and support services
- Provide ongoing patient and family education related to heart failure, device management, and post-discharge needs
- Maintain accurate documentation in the medical record and adhere to regulatory, safety, and ethical standards
- Participate in orientation, ongoing training, and team meetings to support quality improvement
