Job Requirements
- Master of Social Work (MSW) required
- At least 1 year of hospital-based social work experience required
- Experience with psychosocial assessments, safety planning, and crisis support
- Ability to collaborate with interdisciplinary healthcare teams and community resources
- Experience supporting discharge planning, care coordination, and placement coordination preferred
- Strong communication, documentation, patient advocacy, and case management skills
Responsibilities
- Provide psychosocial support and clinical social work assessment for patients in a hospital setting
- Develop and implement safety plans based on patient needs and care goals
- Collaborate with physicians, nurses, case managers, and care teams on discharge planning and next-step placement
- Assist with detox placement coordination and related resource planning
- Document patient interactions, care coordination, and case notes accurately in Epic
- Advocate for patients and families while connecting them to community, behavioral health, and clinical resources
